MIO Memories in Orbit True Ending Walkthrough
This can be done at any point after getting the 4 voices in the main story. In the vault, return to the area where the hand grabs at you and drop down to the space below, go right and follow the path until you find a Severed Fingertip.
Take this to the room above the checkpoint in the lab to repair the mechanism, get the Torn Overseer. You must not get the Overseer from the Vault, go to the big room on the left and into the hidden passage in the drawers. Continue along by activating pumps until you can climb the metal grating, the Overseer will be here.
Return to the newly fixed mechanism and spin it to display 0 degrees then drop down and collect the Torn Overseer. Go back to the mechanism again and set it to 120 degrees. Teleport to the main Vault Overseer and go to the main lift, climb the empty lift shaft to get the to Vaults Shuttle Overseer and activate it.
Return to the mechanism once again and return it to 240 degrees before teleporting back to the Vault Shuttle you just activated and drop down the elevator shaft to enter the Crucible.
The Crucible has two platforming sections to complete with computers at the end. These sections can be the most difficult of the game so take your time and turn on assists if needed.
After engaging both computers, talk to Tomo in the middle of the Crucible and get the Flowing Steps ability, with this you can go to the Canopy and climb the waterfall to reach a new area.
Go right to find another elevator before going down to L-3 Precious Samples to interact with the bridge. Interact again to start the True boss fight. Defeating this will get you the true ending.
